(Photo by Joe Raedle\/Getty Images)<\/p>\n<p>At least a quarter of hospitals in seven states closed their obstetric services between 2010 and 2022, according to a new <a href=\"https:\/\/www.healthaffairs.org\/doi\/10.1377\/hlthaff.2024.01552\" target=\"_blank\">study<\/a> in the journal Health Affairs. And in six states, more than 60% of hospitals lacked obstetrics by 2022.<\/p>\n<p>Using data from the American Hospital Association and federal Centers for Medicare &amp; Medicaid Services, the researchers analyzed obstetric services at short-term acute care hospitals between 2010 and 2022.<\/p>\n<p>The study included 4,964 hospitals.<\/p>\n<p>Rural states saw the most profound losses. The states with the highest percentage of all hospitals lacking obstetrics by 2022 were: North Dakota (73%), Oklahoma (63%), West Virginia (62%), Louisiana (60%), South Dakota (60%) and Mississippi (60%).<\/p>\n<p>The study sheds further light on the nation\u2019s growing maternity health care deserts. At <a href=\"https:\/\/www.cdc.gov\/nchs\/data\/hestat\/maternal-mortality\/2023\/maternal-mortality-rates-2023.htm#:~:text=22.3-,18.6,-Younger%20than%2025\" target=\"_blank\">18.6<\/a> per 100,000 live births, the U.S. has a maternal mortality rate far <a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2024\/jun\/insights-us-maternal-mortality-crisis-international-comparison\" target=\"_blank\">higher<\/a> than other high-income nations. The federal Centers for Disease Control and Prevention has <a href=\"https:\/\/www.cdc.gov\/maternal-mortality\/php\/data-research\/mmrc-2017-2019.html\" target=\"_blank\">reported<\/a> that 80% of maternal deaths in the U.S. are preventable.<\/p>\n<p>The \u201csteady march of closures\u201d has been an ongoing problem, said Katy Kozhimannil, University of Minnesota professor and lead author of the study.<\/p>\n<p>Hospital leaders and experts have warned that Congress\u2019 <a href=\"https:\/\/stateline.org\/2025\/06\/11\/expensive-and-complicated-most-rural-hospitals-no-longer-deliver-babies\/\" target=\"_blank\">Medicaid<\/a> cuts could have a ripple effect, leading to many hospitals no longer able to afford their labor and delivery units, which are expensive to maintain.<\/p>\n<p>\u201cThose programs are a primary source of support for maternity care,\u201d Kozhimannil said. \u201cWithout changes to maternity care financing and organization, I expect that the trends that we have seen over the past couple of decades will likely continue.\u201d<\/p>\n<p>A <a href=\"https:\/\/www.healthaffairs.org\/doi\/10.1377\/hlthaff.2024.01552#:~:text=rural%20areas%2C%20with-,twelve,-states%20(25.5%C2%A0percent\" target=\"_blank\">dozen<\/a> states saw a quarter or more losses of obstetric services among hospitals in rural counties, the study found.<\/p>\n<blockquote class=\"wp-embedded-content\" data-secret=\"X2PAz8LOah\">\n<p><a href=\"https:\/\/missouriindependent.com\/2025\/06\/17\/expensive-and-complicated-most-rural-hospitals-no-longer-deliver-babies\/\">\u2018Expensive and complicated\u2019: Most rural hospitals no longer deliver babies<\/a><\/p>\n<\/blockquote>\n<p>Across rural counties in Pennsylvania and South Carolina, close to half of hospitals \u2014 46% \u2014 lost obstetrics between 2010 and 2022.<\/p>\n<p>Those two states saw the <a href=\"https:\/\/www.healthaffairs.org\/doi\/full\/10.1377\/hlthaff.2024.01552#:~:text=39.7%C2%A0percent)%20saw%20the-,highest%20percentage,-of%20hospitals%20that%20lost\" target=\"_blank\">highest<\/a> percentage of hospitals losing obstetrics, followed by West Virginia (about 43%), Florida (40%) and Iowa (nearly 40%).<\/p>\n<p>Along with Washington, D.C., <a href=\"https:\/\/www.healthaffairs.org\/doi\/full\/10.1377\/hlthaff.2024.01552#:~:text=28.6%C2%A0percent)%2C%20and-,urban,-counties%20in%20Kansas\" target=\"_blank\">urban<\/a> counties in four states \u2014 Hawaii, Kansas, Oklahoma \u00a0and Rhode Island \u2014 saw at least a <a href=\"https:\/\/www.healthaffairs.org\/doi\/full\/10.1377\/hlthaff.2024.01552#:~:text=percent)%2C%20and%20Hawaii-,(25.0%C2%A0percent),-saw%20the%20highest\" target=\"_blank\">quarter<\/a> of hospitals lose obstetrics.<\/p>\n<p>By 2022, two-thirds of rural hospitals in eight states (Alabama, Florida, Illinois, Mississippi, Nevada, North Dakota, Oklahoma and West Virginia) were without obstetric services.<\/p>\n<p>Three states \u2014 Delaware, Utah and Vermont \u2014 had no obstetric losses between 2010 and 2022, the authors wrote.<\/p>\n<p><em><a href=\"https:\/\/stateline.org\/\" target=\"_blank\">Stateline<\/a> is part of States Newsroom, a nonprofit news network supported by grants and a coalition of donors as a 501c(3) public charity.
