Of those, 91.9% were classified as procedural terminations, meaning the Missouri Department of Social Services could not determine whether they were still eligible because of missing forms, incomplete information or other paperwork problems.<\/span><\/p>\n<p><span>That has healthcare and legal advocates worried as Missouri prepares to implement new federal Medicaid work requirements and twice-yearly eligibility reverifications beginning Jan. 1, 2027.<\/span><\/p>\n<p><span>The new requirements, included in the One Big Beautiful Bill signed last year, apply to adults covered through Medicaid expansion. Advocates were already concerned that eligible Missourians would lose coverage because of paperwork they didn\u2019t receive, did not understand or that never got processed by the state.<\/span><\/p>\n<p>        <a href=\"https:\/\/missouriindependent.com\/donate\/?oa_referrer=midstorybox\"><\/a><\/p>\n<div class=\"donateContainer\">\n<div class=\"donateTextContainer\">\n<p>YOU MAKE OUR WORK POSSIBLE.<\/p>\n<\/div>\n<div class=\"donateButtonContainer\">\n                <button>SUPPORT<\/button>\n            <\/div>\n<\/div>\n<p>        <\/p>\n<p><span>Now they say a rule released last week by the Centers for Medicare &amp; Medicaid Services could require additional paperwork as part of Medicaid applications and renewals, potentially causing more procedural terminations and processing delays across the state\u2019s Medicaid system.<\/span><\/p>\n<p><span>The rule narrows an exemption from work requirements for people considered \u201cmedically frail\u201d \u2014 a category patient advocates had hoped would cover people with acute illnesses that limit their ability to work.<\/span><\/p>\n<p><span>But while other exemptions would be met just by falling into a category like being recently incarcerated or getting treatment for a substance disorder, to exempt someone as \u201cmedically frail,\u201d states would have to determine both that someone has a qualifying condition and that it \u201csignificantly impairs the individual\u2019s ability to comply\u201d with work requirements.\u00a0<\/span><\/p>\n<p><span>Emily Kalmer, a lobbyist for the American Cancer Society Cancer Action Network, said she worries cancer patients will lose coverage or experience interruptions in care because of the rule.\u00a0<\/span><\/p>\n<p><span>\u201cWhen you\u2019re going through something like cancer and you\u2019re fighting for your life, now you\u2019re saying that the thing that you need to help you get the treatments that you need is going to be bogged down with even more paperwork,\u201d Kalmer said. \u201cThe state has been having challenges processing the paperwork they get right now.\u201d<\/span><\/p>\n<p><span>States are required to attempt to renew Medicaid coverage using information sources already available to them before requesting information from enrollees \u2014 a process known as ex parte verification. But Missouri only conducted 56.5% of renewals that way in the last quarter of 2025, <\/span><a href=\"https:\/\/ccf.georgetown.edu\/2025\/08\/28\/tracking-state-readiness-to-implement-hr-1\/\" target=\"_blank\"><span>according<\/span><\/a><span> to the Center for Children and Families at Georgetown University. That\u2019s close to the national average.<\/span><\/p>\n<p><span>When the department needs more information, enrollees receive non-renewal letters, including a form they must return within 30 days.<\/span><\/p>\n<p><span>Baylee Watts, a spokesperson for the department, told The Independent in an email that the primary causes of procedural terminations are \u201cmissing information or required actions not being completed, such as the renewal form not being returned and requested verification not being provided.\u201d<\/span><\/p>\n<p><span>But Tim McBride, a professor of public health at Washington University in St. Louis who previously served on Missouri\u2019s Medicaid oversight board, told The Independent recipients don\u2019t always receive the form if they don\u2019t have a physical address or if the address on file with the department is incorrect. And he said it\u2019s not always clear to enrollees who receive the form what the department needs from them.<\/span><\/p>\n<p><span>\u201cI\u2019ve seen the letters, and they\u2019re really complicated,\u201d McBride said. \u201cThey\u2019re kind of abstruse, to be honest with you\u2026. We\u2019ve heard from someone, \u2018I thought it said I still had Medicaid.\u2019\u201d<\/span><\/p>\n<p><span>In 2025, Missourians were dropped from Medicaid at renewal at a higher rate than throughout the U.S., with a lower percentage keeping coverage and higher percentage left pending per month, <\/span><a href=\"https:\/\/timothymcbride.substack.com\/p\/missouri-medicaid-update-2025-on\" target=\"_blank\"><span>according to McBride<\/span><\/a><span>.\u00a0<\/span><\/p>\n<p><span>Kate Holley, a senior attorney for Legal Services of Eastern Missouri who assists clients with public benefit applications, told The Independent the letters sometimes list incorrect reasons for enrollees\u2019 pending non-renewal.<\/span><\/p>\n<p><span>\u201cThe reason that\u2019s listed may not even be accurate,\u201d Holley said. \u201cIt\u2019s just what the system pulled. Often there\u2019s several, and not all of them are correct. So really, a person needs to parse through all that and figure out, \u2018What do I need to do here? What do you want me to do?\u2019 And that\u2019s before even trying to engage with the agency.\u201d<\/span><\/p>\n<blockquote class=\"wp-embedded-content\" data-secret=\"IdvFwFIidY\">\n<p><a href=\"https:\/\/missouriindependent.com\/2024\/06\/17\/feds-to-scrutinize-missouris-worst-in-the-nation-medicaid-application-delays\/\">Feds to scrutinize Missouri\u2019s worst-in-the-nation Medicaid application delays<\/a><\/p>\n<\/blockquote>\n<p><span>The department has drawn scrutiny from the Centers for Medicare &amp; Medicaid in the past for application processing times in excess of the 45-day federal maximum. Federal officials\u00a0 <\/span><a href=\"https:\/\/missouriindependent.com\/2022\/08\/09\/federal-government-steps-in-to-help-pull-missouri-out-of-medicaid-backlog\/\"><span>initiated<\/span><\/a><span> a mitigation plan in July 2022 after processing times reached 116 days that June and stepped in to conduct a <\/span><a href=\"https:\/\/missouriindependent.com\/2024\/06\/17\/feds-to-scrutinize-missouris-worst-in-the-nation-medicaid-application-delays\/\"><span>\u201cfocused review\u201d<\/span><\/a><span> in 2024. The department\u2019s numbers had improved but crept back up, so that in February 2024 72% of applications took more than 45 days to process.<\/span><\/p>\n<p><span>In February 2026, about a third of applications took more than 45 days to process, compared to 7% nationwide.<\/span><\/p>\n<p><span>Even when recipients return the form \u2014 listing their employment, earnings and assets like cars, and attaching paystubs \u2014 advocates said they can lose coverage because the paperwork doesn\u2019t get processed.<\/span><\/p>\n<p><span>The department\u2019s computer system sometimes automatically terminates clients\u2019 coverage if staff don\u2019t begin processing their paperwork before the 30-day deadline, said Joel Ferber, director of advocacy at Legal Services of Eastern Missouri.<\/span><\/p>\n<p><span>\u201cWe have all these clients that send in all their paperwork, and it doesn\u2019t get processed, but the system closes the case,\u201d Ferber said.<\/span><\/p>\n<p><span>Part of the problem, Holley said, is department staff have to complete much of the process manually.<\/span><\/p>\n<p><span>\u201cSomebody has to take it out of the portal, look who submitted it, file it in that person\u2019s electronic file,\u201d Holley said. \u201cAnd if [the recipient] does [the paperwork] a couple days before they\u2019re set to close and then they\u2019re two weeks behind, that person\u2019s going to get auto-closed, auto-terminated.\u201d<\/span><\/p>\n<p><span>The department has been trying to <\/span><a href=\"https:\/\/missouriindependent.com\/2025\/11\/24\/federal-changes-delay-long-overdue-overhaul-of-missouris-troubled-safety-net-systems\/\"><span>automate more of these processes<\/span><\/a><span> ahead of the January federal deadline to implement Medicaid work requirements, saving staff \u201cmanual touches.\u201d<\/span><\/p>\n<p><span>But experts worry the new rule from the Centers for Medicare and Medicaid could interrupt attempts to streamline processing.<\/span><\/p>\n<p><span>Tricia Brooks, a research professor at the Georgetown Center for Children and Families, told The Independent that states have been preparing to automate exemptions such as the one for medical frailty using diagnosis codes for qualifying medical conditions.
